Change of Address

To change your address registered with the practice, you will be required to complete a change of address form. These can be obtained from either North Thoresby or Holton-le-Clay. You can also update your details online via SystmOnline if you have access to this.

Patients who are changing their address to an area not covered by the practice will be informed they will need to register with a different GP that covers their new address. We usually give a grace period of up to 2 months before NHS England are informed and will remove you from the practice register.

Please be aware if you have moved out of the practice area, we are not able to update your address to your new address and you will remain registered with your previous address until you are registered with another practice.

Change of Name

To change your name, we will need to see proof of this. This can include a marriage certificate, deed poll, divorce decree, decree for the dissolution of civil partnership or court order.

A copy of the document does not need to be taken as we only need to state we have seen the document to inform NHS England. Once this has been seen, your name can be changed and a notification is sent to NHS England to authorise the change. You will be contacted by the practice if there are any problems.

Incorrect Personal Details

There may be some occasions that your other details are incorrect. Please ensure that telephone numbers are kept up to date with the practice so you can be contacted if required.

If there is a correction that needs making to your date of birth or title, please provide documentation that show the correct information. This can include a passport, birth certificate or court order. A copy of the document does not need to be taken.
