You have the legal right to request a copy of the information we hold about you, in line with the General Data Protection Regulation (GDPR).
You can request this information by obtaining a form from the Care Navigation team at either North Thoresby or Holton-le-Clay. On returning the form, you will need to show two separate forms of identification – photographic and proof of address.
If you are requesting this on behalf of someone else, you must have consent on the records for the patient you are requesting for and provide further identification. Details of what documents are accepted are detailed on the request form.
Once requests have been received, they will be actioned within 20 working days. Requests that require paper records may take longer as all patient paper records are kept off site. If further time is required to process your request, you will be informed of this.
Once your request has been processed, you will be contacted to collect your records from your preferred site. Photo identification is required on collection. Without seeing this, we cannot release the records due to the sensitive content.
