Eligibility

Who can be referred?

• Be Smoke Free - Over 12’s who are currently smoking tobacco

• Eat Well, Lose Weight - Over 18’s with a BMI of 30+ (BAME 27.5)

• Drink Less - Over 18’s drinking more than 14 units of alcohol a week

• Move More - Over 18’s doing less than 150 minutes of moderate intensity activity a week.

Eat Well, Lose Weight

We’re all unique! This service has something to suit everyone, with a wide range of options all designed to help you eat more healthily and lose weight.

Be Smoke Free

Helping you to quit smoking, our 12-week stop smoking programme means you’ll be 4 times more likely to quit, for good. They will look at your history, help you set goals and offer follow-up support and Nicotine replacement therapy (NRT).

• Weekly support in our 1-to-1 or group sessions.

• Flexible appointment times to suit you, including evenings.

• Tailored support to help you break habits and boost your motivation and confidence to be smoke free.

• FREE Nicotine replacement therapy.

• Helpful resources like our Quit Guide.

• Check in contact at 6- and 12-months smoke free.

 

Drink Less

Everyone Active’s GP referral

Everyone Active’s GP Exercise Referral scheme is the perfect way for people with long-term health conditions to move more to improve their health and wellbeing. We want everyone to be able to enjoy at least half an hour of physical activity, five times a week, and we’re here to help you.

The 12-week programme will be designed to help individuals self-manage their condition more effectively and with more confidence. Moving more will get you on the path to a happier, healthier you.

 

Who can be referred onto the GP exercise referral scheme?

Our Patients living in North East Lincolnshire and most people with long-term health conditions will be accepted onto the GP Exercise Referral Scheme. These include asthma, arthritis, high blood pressure, diabetes and obesity, as well as depression, anxiety, osteoporosis and plenty of other conditions, including rehabilitation from Covid-19.

The programme has been designed to help individuals self-manage their condition more effectively and with more confidence.

 

How you can join the scheme:

Simply book a telephone appointment through our care navigation team to speak to a GP.

 

What does the GP referral scheme involve?

At an initial appointment the referral instructor will find out what is important to the individual and help them to find activities that they will enjoy! A 6 and 12 week review will enable the instructor to support the individual through their first 12 weeks to a healthier lifestyle.

Activities vary at each centre but may include:

• Walking Sports or groups

• Chair based exercise

• Swimming

• Group exercise classes

• Water based activities like Aqua Fit

• Gym-based exercise
